A doctor-patient relationship is established only after an in-person consultation and examination and our mutual agreement to begin care.

4. Medical Emergencies

Do not use this Site to communicate urgent medical concerns. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.

5. Appointments and Communications

Appointment requests submitted through the Site are requests only and are not confirmed until you receive confirmation from our office. We reserve the right to reschedule or cancel appointments and to establish policies regarding missed appointments and late cancellations, which will be communicated to you as a patient of the practice.

Communications sent through the Site (such as contact forms or email) may not be secure and are not monitored continuously. Please do not include sensitive personal health information in messages sent through the Site. For matters involving your care, please contact our office by phone.

6. Privacy

Your use of the Site is also governed by our Privacy Policy, which describes how we collect, use, and protect information submitted through the Site.
